Respected Sir Madam,
I, Jayaraj M. Nadar would like to bring to your kind notice that on 8th March I have received an Insurance Investment document from Bajaj Allianz Invest Plus. Policy No. 0155096685. I, had never accepted any offer or shown any inclination for the Bajaj Insurance policy.
The policy states that I had applied for the Life insurance on 02nd March which isn't true. The company or the insurance broker has even went to the extent of getting my xerox copy of the passport from somewhere- however some of the info in the passport xerox is forged..example:- Date Of Birth.
Along with this they have also attached a copy of the application form in the policy where the following information is incorrrect.
1) Father's Name
2) date of birth
3) Address:-
4) Mother's Date of birth
5) Profession
6) salary
7) Language
8) Cell no.
9) Bank details
10) account no.
11) age
The most important part is that the application also contains my forged signature which was really disturbing to me and it has resulted in insecurity and loss of trust in a reputed company like Bajaj Allianz.
I would highly appreciate if the concerned authorities could please initiate a severe action against the insurance broker (Rajendra N Borade--as mentioned in the policy document) and Bajaj Allianz Life Insurance Company.
