Application form is lost

Dear Sir

I Saket Raj Tripathi working as Software Engineer. I have an account in Bank of Baroda, Jalalabad Shahjhanpur U.P.-242221. I have applied for ATM card and Internet banking on last week of Dec'09. Yesterday I inquired about it, Bank staff including Manager said that your application form is lost some where and we are looking into this matter.

Please consider this for the above mentioned regard.
