Replacement ATM Card Not received From Last 4 Months

Dear Sir /Madam,

Dear Sir/Madam,

I am sandip Bahausaheb Pabale. I have my Salary Account (Account Number:-050210110000993) into your Bank (Branch HADAPSAR INDUSTRIAL ESTATE, POST BAG NO. 1, PIN:411 013 City: PUNE). I have lost my ATM cum Debit Card & I have been deactivated Card through Hotlisting Services.

After that I had applied for new Debit Cum ATM Card Dated on 10th Sept.as per bank policy user must receive new ATM Card / Pin within 15days time spam.in-spite of taking lots -of followups through Verbal, Telephonic and Mail But unfortunately still I’m not received PIN nor ATM Card. So, I kindly requested to you to Please help me on this issue.
