This refers to above mentioned policies I would like to draw your kind attention about cheating, misleading and fraud have been done with me by your company's one of the manager Mr. Akhilesh Jha and Mr. Ashok Kr. Shukla, of your Vasant Vihar Branch office not once but twice.
Firstly he cheated me on 16.09.2009 by saying that I should give him a cheque of Rs.3000.00 he will upgrade my policy no.500-4073507 dated 21.08.2009 from Rs.10000.00 to Rs.16000.00 anual (he has Rs.3000.00 of my commission with him). I came to know when my policy no.500-4243647 Dt.18.09.2009 came to me, that he has make another policy without my concent or my signatures.
Next time I have been mislead or misguide when he has taken a cancelled cheque from me by saying that for attaching the upgradation of amount he need cancelled cheque. I was utter surprise and shocked when I saw my bank statement, it shows that from September onward your company is taking premium of Rs.1501.01 by ECS, when I have not given any instructions for ECS.
Can I request to your goodself to please look into these matter, and ask your representative how he has made my next policy without my concents and fourged signatures, and also ask him why not he upgrade my first policy after taking the full amount from me. I have returned my both the policies to Mr. Akhilesh Jha for cancelation.
Now, I have decided, after see the atitude and boldness for doing cheating and fraud of your manager level man, i don't want to continue my above mentioned policies. So you are requested please cancel my subject policies and send me the amount by bank draft on below mentioned address at the earliest. Otherwise I will have no option left to go court of law.
