ATM CARD RE-ISSUE

Dear Sir/Madam,

I lost my ATM card befour four mounth. Few times I have personally visited with concern branch manager and operation manager of the branch but nothing has happened till date.

This is not acceptable from a reputed Govt. organization.

My S.B.a/c no is 3057203232 with Habra branch, West Bengal.

Requesting you please try to solve my problem as early as possible.
