Dear Sir,
This is to inform you that, I have an E.P.F account no. MH/120527/80 with you and my company ECOLUTION OUTSOURCING PVT LTD having Establishment Code No. MH/120527. I had joined this company in June. From the date of 1 Jan 06 this company was deducting my P.F regularly till August. Now I had left this Job in august 08. Company has regularly deposit my P.F in my P.F account and I had submitted my P.F with drawl form in your office with signed by Mr. Deep (Director of ECOLUTION OUTSOURCING PVT LTD).
I am continuously followed up with my company, but they are not confirming anything else. Even I am not able to view my account balance on your website. I am enclosing the P.F slip which is provided by you for your reference.
