Dear Sir,

With reference to the above I would like to draw your kind attention that I am having above saving a/c no. with your branch since February and would like to inform you that the I had submitted the pass book for entry of transactions on 19/08 but I came to know that a false entry made for Rs.15000/- on 13/8 with transaction no. PRCR/81007152/92 which was not made by me and I assure that I have not made any purchase on this date.

Hence I request kindly look into the matter and arrange cr. for this amount immediately and confirm.
