i myself sanjay kumar das was working in icici bank limited kolkata in R.B.D. Department as a Business Development Executive.at sarat bose road branch, and my reporting boss was sales manager[kjshor jha] who is sitting jn sarat bose rd branch.and that time A.S.M.was pankaj chawla.

i was working there between the period of August to November.my employee code was-505998, and my salary came into my a/c no-037201500899, cust id-508424428 at SARAT BOSE ROAD BRANCH KOLKATA.

i earlier sent letter to the icici bank main branch kolkata, and P.F.Department to transfer my P.F.amount to my new a/c S.B.I.JAIJAIPUR CHHATTISGARH -30561942676 OR issue a cheque of such amount and send to my home address - M.I.G.-50, PADMNABHPUR NEAR STADIUM DURG [C.G.]
