Non receipt of cheque book

Dear Sir,

Around a month back, I indented for issue of cheque book by dropping the same in the box provided at ATM at Shalimar Square, Hazratganj, Lucknow. But I have not so far received the cheque book. Please look into the matter and arrange to send the cheque book at the given address immediately.
