Dear sir, I had purchase the above noted policy commencing 28/12 and paid a premium of Rs.100, 000/- (Rupees one lac only). I was explained by the visiting agent Dhesi Nidhi (00012764) that this is one time premium and the maturity proceeds would be paid to me after three years. Now, when I approached your Phase-9 branch for payment of the same, I was refused payment stating that the premium in my above noted policy is payable yearly. As I had not gone through the policy document and relied on the explanations made by the visiting agent, I had written to your Mumbai Office that they had assured me that the necessary amendments would be made in the policy document. Now that I have been refused payment, it seems that the needful has not been done at your end. You are therefore, requested to cancel the policy and afford me proceeds of the same at your earliest at my address mentioned below. My D.O.B is 29.03.1952.
