Remarkable / purported / motivated delay in respect of Full & Final settlement of: A/c no: WB/12388/1912 & EPF No: WB/12388/P-1243; for P.F. / EPF/ Pension.
Dear Sir,
I am now forced to beg your Intervention and to make this request for my help, as you are Principal & Head of the Authority / Organization /Institution / Employer I hope that you will find it possible to grant it.
I boon to you for necessary immediate action for the following facts and which please be note:-

1 That I Shri Aloke Kumar Chattoraj – an ex-employee of M/s. Kesoram Spun Pipes & Foundries, Prop: Kesoram Industries Limited, 10, Camac Street, Industry House – 12th. Floor, Kolkata – 700 017, has applied for full & final settlement / withdrawal of P.F & E.P.F and Certificate of Pension Scheme under 10C to the Birla Industries Provident Fund (BIPF), 15, Maharana Pratap Sarani, Kolkata – 700 001, about 20 months back.

2 That, the Birla Industries Provident Fund (BIPF) is Trustee Body, where I have joined as a member thro’ Kesoram Spun Pipes & Foundries.10, Camac Street, Kolkata – 17; on December. and worked there upto November. Copy of Statement thro’ prescribed format & acceptance of applications are enclosed.

3 That Initially BIPF was informed their action vides their letter date: April 13. Ref: BIPF/WB/12388/AG/41, but after that they are also kept their selves silent / inactive purposely & motivatedly without any reasons and thus harassed & damages day after days.

4 That I have resigned from M/s. Kesoram Spun Pipes & Foundries Prop: Kesoram Industries Limited, Industry House – 12th, floor, 10, Camac Street, Kolkata – 17, in proper Official Notice & in relate to my terms of employment and accordingly they have released me, after fulfillment of all formalities and there is no reason behind it, except their whims.
My appointment letter / job confirmation letter / Resignation as well as Released Order - all copies are enclosed herewith for your necessary stern disciplinary action against BIPF and Kesoram Spun Pipes & Foundries, Kolkata – 17; these copies are self explanatory.

5 That the PF & EPF money has been transferred from my previous employer M/s. EPCOS Ferrites Ltd. under your B.T.Road, Titagarh - RPFC Office on November 23 - to Howrah SRO PF Office.

6 That, as you are Principal & Head of the Authority / Organization / Institution / Employer in this issues or subjects as mentioned / explained above., also, I have applied to you directly last April 07th. 2009 on your stipulated format – F/19 & 10C at least 16 months back. Neither you have made or acted nor any single reply of my application or any action towards full and final settlement – Acknowledgement receipt is enclosed herewith for your ready reference & action.

7 That, now it is about two years (2) has passed & crossed in BIPF as well as in your good Office, but no action has initiated at your end, besides my health position is now very alarming and gradually deteoriating, hence I need immediate money & to start immediate Pension under your pension scheme for the survival & to avoid starvation of my family.
8 That finally BIPF and the respective authorities will be responsible for any mishaps / harm / injury or any incidents / accident of his family or any member. Also they are liable for unnecessary additional / extra cost incurred in this issues & Mental and physical torture harassment & damages.
9 That I have also reminded you thro’ mail dated: April 02nd. & 03rd. 2009 to you and your HQ- Salt Lake PF Office for necessary action towards his final settlement, but neither any action has initiated for full & final settlement of PF/EPF/Pension.
10 However and finally, this is again reminded / requested / appealed to take necessary action towards releasing the full and final settlement as asked / claimed in your prescribed format F/19 10C & Pension Scheme immediately. As you are Principal of this Organization.
11 That, please be deposit full & final settlement amount to my Banker: ICICI Bank, 21, R.N. Mukherjee Road, Kolkata – 700001 Branch. SB A/c No: 000 601 01 6738, as I am unable to attend personally due to my ill health.
