EMI debited twice

Hi, this is regarding the EMI payments on my LIC housing loan. As per the LIC account statement, I understand that a cheque, given by me in Jan had returned. However, as per my bank statement, the amount has been cleared in Jan. Moreover, there are several entries in the LIC account statement showing returns and then credits of the same amount. Again as per my bank statement there are no such returns at all.

The LIC statement shows 2 PDC's against same month EMI.
