Premium amount is 1269 quarterly.

2) Policy No:800417721

Name: Manthena Jyothi,

DOB : 30/08/1978

Premium amount is 640 quarterly.

3) Policy No:800417742

Name: Manthena Swapna,

DOB : 30/08/1979

Premium amount is 640 quarterly.
