Non transfer of EPF in my bank account

Dear Sir,

I have submitted EPF withdrawl form and two cheques no. 633397 & 038126 of amt. Rs- 9778/- & Rs- 4305/- dated 01/12 were issued by the department. But still not transfered in my account no. S/B- 2834 of The Jammu & Kashmir Bank Limited, Roop Narayan Market, opp old roadways bus stand, Aligarh (UP) 202001.
