Did not submitted my PF withdrawal form to PF office

Dear sir,

I have applied for my PF withdrawal before approx. 6 months to our HR office in Mumbai. and from that day I am not getting any proper responce from mr. Natrajan (executive-pay roll) because I have sent it to him, now he is not responding to my emails and when i called him, he repllied me that its not his job

My details are as under,

when i cheak with the PF office than I came to know that the form has not been submitted yet to the PF office. Please do something regards this problem.
