EPF claim not setteled since one yea

Dear Sir/ Madem,

I have applied for withdrawl of epf amount on 30/9. That time my account number as provided by Office of GMT Raigad at Panvel was MH/BAN/46391/109. On 27/10 after enquiry at Bandra EPFO, I came to know that my account number is MH/BAN/46391/45. But in statements provided by both BSNL Panvel and Aurangabad my EPF account mentioned is MH/BAN/46391/109. After enquiry at BSNL Circle office at Santacruz I found out that my account no is MH/BAN/46391/45. So on 27/10 EPF office asked me to fill up new form for withdrawl and account number mentioned in this form is MH/BAN/46391/45. After submitting this form around one year is passed but neither case is settled nor i am able to know my epf balance amount on website. In know your claim stautus link of EPFO, for account no MH/BAN/46391/45 i get form submitted date wrong as, "11/02". I have hard copy of claim form receipt dated 27 Oct. And for account no MH/BAN/46391/109 i am getting no matching result message.

Please help me out to settle my claim with proper amount withdrawl.
