PF widrawal not received

Respected Sir,

I have submitted my PF withdrawal form (A/C No.-KN/BN/34272/124276) to my employer on 03 January-09 after so much excuses thy have inform me om 18 Nov. 2009 that they have submitted it to EPF comissioner's office Bangalore on 05 Nov. 2009 but till the date no information send to me by the EPFO nor the withdrawal amount.
