Merging of PF account

I have two pf account one for my old emploaye and other for my new employer

1 . old employer pf account number- 10789/25 - indore

2 new emloyer pf account number- mp/ind/0007002/000/0000078 -indore

please merge above account in single account because pf amount hold in old account also

I have already submited for merging form to indore pf office but they told my form are lost and now old employer does not give me new request for merging please suggest me whay i do???
