Not delivery of gas cylinder after repeated requests

I have a gas connection no. 641750 in the name of smt chanderwati

I had booked the gas vide booking no 427795 dt.5.3.2011and no.467953 dt.6.4.2011. Inspite of reapeted requests the cylinder has not been supplied at my resident at c/o Meharchand 289 tigipur delhi. 36.please take action as earliest.
