No sending application form after taking demand draft of 950 rupees

Sir, i have sent the registration form along with demand draft of 950 ruppes according to condition of nchmct 2010 by speed post on 8/02.But not sending OMR Application form of nchmct 2010 yet 01/04, and 12/04 is last date of reciept application form.
