As per your letter no. MH/PF/KND/ACCTS/90687/65/1591 dated 29.03.10, i have resubmitted following documents on 12.04.10.

1. Xerox copy of my saving bank pass book with photograph /address duly attested by branch manager.

2. Explanation regarding delay in submission of claim forms.

Although i waited for past 3 months i didn't get any response from your office.

Kindly do the needful. It is already delayed for more than 5yrs.
