I the undersigned Ananad Dinkar Kulkarni, was working with Aksigen Hospitalcare Pvt. Ltd., a pharmaceutical company as Area Sales Manager from 3rd Aprill 2009 to 12 th July. The address of the said company is as follows.

The above said company sent me the form for P.F. claim and withdrawal on my address and I sent that form to the company with all the details from my end and confirm telephonically that they recieved the form. After that, from last 45/50 days I am calling the perticular person of that company to submitt my form to the P.F.office with the details from the company end. But unfortunately, that person is not picking up my phone till date and my one friend of Mumbai done the enquiry @ my P.F. claim status at Bandra office. But he came to know that company is not submitted my form and documents in P.F.office yet from 45/50 days.
Sir, I am not in any job from last 6 months, and I want to withdraw my P.F.amount. I do have the family financial responsibilities and my son is also giving the entrance exam of National Defence Acadamy, Pune. For all these things I am in very much need of money. I am not asking to settle any claim from company. I am asking for that money which company deducted from my salary as P.F. contribution for 14 months.
So, this is my humble request to you to look in this matter on humanity ground and give me justice.
