PF amount not received in six monthes

Dear/Sir Madam,

I left above company in june I have sent my forms through my former superior in sep. 2008 & company had confirm the receipt of docs but the company has not yet given my PF amount and I wish to withdraw my P.F amount. Please suggest if there is a way out.
