They lost my atm application form with photo

I opened a account in canara bank last december, that time i applied for atm card, that card is not get now, my mum went that bank last week at 11 am, they are not give good response, they ask to come after 3.30pm, my mum went another time, they asked to my mum that atm form is not here, say to apply another form for atm card, my mum is 65 aged, they are not give good response to senior citizen.
