Problem In Salary A/C Opening

I have salary a/c (05721050035886) my customer id is (27781055) in HDFC Bank Ltd. Vatika Atrium, Sec-53, Gurgaon-12 my A/C was opened in this branch in 20-11 . But i have not received my welcome kit till now. Many time i visit in this branch & Two times i have given my id proof (Votar ID) in this bank but i am not gate any positive response from this branch & i have not receive my salary from last two months so please give me the positive response.
