Non issue of Debit card

Sir,

This is to inform you that iam operating my account (No:14108, Eightfeet road, KK nagar

Madurai, Tamil nadu) for past One & half years. During the shift of residence i lost my Debitcard,

I intimate the same to the bank more than four times from Jan to June & requested for duplicate

Debitcard, but nothing happened so far. Hence i request you to look into the matter & solve the long

pending issue as early as possible.
