Sub: My FDR No. FD 151236 Dt. 13.04.2000 matured on 12/04 for Rs. 21304/-

In spite of my several reminders & request, I have not yet received the maturity amount of the above FDR .

I have already submitted the original certificate to their Kolkata Office on 30th April for getting FDR maturity amount.

We require your help to get the maturity amount,
