Dear Sir,

Please note that I have been applied for the Voter Identity Card through proper channel but some how my name have not been appeared in the list declared recently at our Dum Dum constituency.

I have submitted all the documents along with application Form – 6

Hearing date was : 10-08

Place of hearing was: K K Hindu Academy, Motijheel, Dum Dum, Kolkata

Receipt copy of the Form – 6 signed by: Ms. Renuka Biswas

Receipt copy of the Form – 6 no. mentioned : 1082 (1871) 1/91 Elec/BKP

AC No : 114, AC Name: Dum Dum, Part No. : 125

Please advice and inform how to get the voter ID Card immediately
