I had recieved my withdraw to my Bank Account

Sir,

I had an withdraw from my I, D. WL.63571 on 30th September of Rs 3570, but till yet I had not recieved in my Account, for that I had made correction for so many times to the concerned Department, but every times they make mistake & I had to victim off, So I here by sending my details once again please do the needful for my withdraw, So that I can had my another withdraw My details are below.
