I worked in the establishment M/s. Empee Offset Pvt. Ltd., A41, Royal Industrial Estate, Naigaum Cross Road, Wadala, Mumbai – 400 031, baring Code No. MH/44419/47 for past Four years, but my ex-employer has refuse to adjust my claim Form & not paid the proper contribution from Jun-06. I have made various requests on telephone, but the employer does not care of it. In this regards I am requesting your good self to take necessary action to forward the PF claim.

I was resigned from job on 19 Jul, till date I have not received my PF claim from ex-employer.
