Employee Provident Fund Office Bangalore Complaints - my Employee Provident Fund not yet settled by Employee Provident Fund Office, Bangalore

Dear Sir,

I worked with Les concierges services Privet Limited (Head office Bangalore) at Delhi as a Desk executive, I had resigned 24th April and I had completed my resignation formalities & and all the papers send to Employee Provident Fund Office, Bangalore through my Employer. After 3 Month I received my form back by this office and application details are mention below :

Please fill citi bank branch address.

I fill new form and send it back to Employee Provident Fund office by my Employer & again after three months I got my Employee Provident Fund form back.application details are as below,

Your account has been already settled.

My Provident Fund No. is - 24227/1597

Kindly look in to the matter and do the needful.

I am uploading receiving paper of my form dated 2007 from Employee Provident Fund.
