MY EPF withdrawl cheque got made in wrong a/c and I am trying to have the new cheque made from the EPF, KR Puram Office, bangalore.

I had called the office no 080-25611955 nearly 15 days back and was told I need to fill out reauthorization form. I now live in Gurgaon and the EPF office has no such form. I was asked to furnish the request on a plain paper by the Gurgaon office official.

Since then I have tried to speak with the KR Puram office on the above mentioend no. but the number is constantly engaged during the office hrs and its only after office works that it goes through... obviously by then there is no one to take the call.

I have sent around 10 mails with the reauthorization request but just no reponse... Email communicaiton is consiered approved method of comunication.

I need my withdrawl amount urgently.
