PF form rejected

Dear Sir/Mam,

My PF Number is: KN/24621/219. This was submitted for withdrawal by my office on the 18th of July and the same was rejected on the 9th of September. But I have still not received the rejected form and hence do not know the reason for rejection.

The same cannot be re-submitted without knowing the reason.

Request you to help me get the reason so that the corrected withdrawal form can be submitted to get the PF amount.
