Subject: Regarding PF Claim as the company is closed & not being found.
Respected Sir,
With due regards I am hereby sending you my PF Withdrawal
Form 19 duly attested by my Bank Manager as per instructions of Central PF Commissioner – Delhi.
My PF A/c No is GJ/ 18651 / 11561 and I was working in M/s; Fanal Security & Allied Services Pvt Ltd, 401, 4th Floor, Pawan Plaza, Sardar Patel Colony, Narainpura, Ahmedabad, since 06.12.2002 till 29.02.2008 and the company winded up & we submitted our PF Withdrawal Form to them but no action was taken and we are unable to trace them now. Then I contacted Central PF Commissioner Office – Delhi & he told me to send the PF Withdrawal Form duly attested by Bank Manger where I have my Bank Account and to send you the same with letter.
Now I am sending you the same and request your kind honor to please help me in getting my PF Money & I am attaching the Photocopy of My PF Deposit Slip along with this form.

I had send my EPF Form –19 Duly filled & attested by my Bank Manager to Regional PF Commissioner Office – AHMEDABAD on 15th Dec’2008 through Courier which was received there at AHMEDABAD PF Office on 17th DEC’08 but I am sorry to say that till date nothing has been done in this regard & my PF Claim is still in Pending & Unsettled. I am in need of money very urgent to meet my day to day needs, hence I request your kind honor to please help me out and get my problem solved I will be grateful to you sir.
