Not received my relieving date letter till date

Dear Concerned,

This is to bring to your notice that i worked with HBL Global Pvt Ltd till May '06, till date i have not received my relieving letter.

Please look into the matter and revert back asap as i need to produce these documents in my organization and confirm with whom i can be contact for resolving this issue. My Emp Code was M01783.
