Unwanted bill

It is to inform you that i m having a master gold credit card number 5176521007074620 on the behalf of name of Rahul Kaushal.

This month i got bajaj allianz medical policy from hdfc on the behalf of name of Rahul Sharma for Approx 1800Rs and the monthly emi, policy charges are included in my bill statement. i did not make any request for medical policy. After recieving the policy i went to your hdfc feroz gandhi market ludhiana and they told me to talk with customer care then i called to customer care two times and they told me that they will reply within two days but now its more than one week

you are please requested to generate a new bill and send it to me & also find out the who is the employee of ur company who has done this
