Hdfc sip, tax plan
Not getting account statement

Dear sir/madam,

my self sangram kesari swain, had done hdfc sip plan from a agent AVINASH HIRABHAI RAVAL, lla-llu feelings, cosy tower, palanpur, gujrat (e mail id ill-ilu [email protected]).my folio no. is 5540799/08.in my policy he had registered his above mentioned e mail id.i have no contact with him now.so i am not getting my account statement&other relative statement.my e mail id is [email protected] which, i want to be registered.every month Rs.1000 is deducted from my salary from hdfc bank.pl. help me.
