About 2 nd Cylinder issuance

I have one gas cylinder. we applied for the 2 nd gas cylinder in March. Till now we did not get cylinder. each time when we are asking in agency, they are telling that it has not come, some times as it was cancelled. Can you please explain me the follow up procedure
