Poor response from customer care

Hi,

My issue is pending since more than 2.5 months and has not been resovlved despite of several dates commited by customer care and ICICI bank managers. Need to have contact details of banks top officials. Can some one provide me the mail ID of Mr KV Kamath or MS Chanda D Kochar.
