Non Closing of account

I have sent my all documents along with application for closing of Saving as well as Demat account to Bhubaneshwar janpath branck and i have a receipt copy (4/12) for the same but inspite of writing may times and clearing all dues my account is not yet closed and they are imposing penelty on me of non maintaining minimum balance. kindly help me in closing of my account.
