Pending PF withdrawal

My self Apexa Shah I was working with ur organisation from August to March due to some reasons i left the job. I was working with IFFCO TOKIO GENERAL INSURANCE SERVICES LTD.as customer care associate at L3 level ITIS (5537), DEP ID:64855.My Employee ID : 82785. According to ur contract of employment i have contributed (Employee's contribution) provident fund for 09 months ESI.so i m requesting u to please tell me the procedure how i can get back my contribution and please clear all my dues as soon as possible.
