I am a resident of New Tilak Nagar, Mumbai-400 089. The details of my Assembely Consituency & Election List is as follows:
-Assebelly Const. No. : 174-Kurla (st/sc)
-Election list No. 32
-Sr. No. in election list : 211
-Form No. & Date of submission of EPIC -001B Form: 052262 dated 24/06
Visited the concern Election Office at Kurrka, Mumbai- for more than 13 times. Each time they are asking me to visit after 15-20 day. Numer of visits have also been made to the Senior Election Officers at Collector's Office, Bandra, Mumbai-, but all attempts are in vain. No one is held responsible or answerable.
Please look into the matter & get me my Voter's Photo Idendity Card (EPIC)& also advisse me in the matter.
