Non Receipt of Form No. 23

Dear Sirs,

We not received form no. 23 for our establishment (Code KN / 23529) from 06-07, 07-08 & 08-09.

In this regard I met several times to consern officer but all my effort gone vain.

Our employees are asking their annual statement of contribution (Form 23) day by day. My humble request is take some adequate action and send the same immediately.
