EPF amout not Withdrawl on my account

Dear Sir/Madam

I would like to inform you that I had paid my epf withrawl form No.19, 10c and last year 3A sheet and my recipt no.is 2816559 dt.16/12 and EPF A/c. No. HR/GGN/10664/0920.when i had paid this form then i reicet a counter slip. In this slip u have write last date 15/01 of clearance of my EPF account. But now till date I have no receipt any amount by EPF Deptt. So Please Kindly take a action for my epf account.
