PF FORMS NOT FOUND

Sir,

I have submitted my PF withdrawl form alongwith 5 members with PF Office, Wazirpur on 12.1.2011 myself. I have the acknowledgement receipt for the same. Now I contacted PF office, south at Dwarka, they informed that they have not found the slaim form.

Already two months have been elapsed, and i m in dire need of month. You are requested to kindly help me/us for finding out the forms.
