Passport not Received Yet

I have applied for my passport since july'07. Actually i lost my reciept, so that i am confused about my my file no either it is Z001516 or Z001519. I checked the online status, initially it used to say "Information is not available". though i have submitted all the required documents still i have'nt received my passport. it is delayed for more than 15 months. kindly look into this matter and issue my passport without any further delay as i'm going to travel abroad in the near future.
