I m satish kuamar sharma want to inorm u that my pf no i s KN/6703/8295.

TWO cheque had been issued in my account but only one cheque's ammount had been released.

second cheque no 59118 dt 16-10 amt 18281 is not released yet.

after communication at pf office at bangalore i was told that i need to fill up reauthorization form again.

i want to know that is any particular name of this form or i need to send the same xerox copy of earlier form

plz suggest me
