Dear sir,

I have Monthly income scheme account at Rajkot Head post office, Gujarat. Following books are matured but post office refused to give me maturity amount. The post master has misplaced my application form and no physicial form copy available at Rajkot post office.

A/c Book no. 111541, 111542, 111543, 111544.

Kindly take a necessary action for post master to misplaced my original hard copy of application form.
