Dear Sir,

My mobile no. is 9839122111. I had applied for new post paid connection on 28.07.09 through your DSA Koosh Yash Communication, Munshi Pulia, Lucknow. I discussed with your representative for PLAN 400 Dobule and filled the same into the form & when i got the information about my bill they have applied a plan 699 instead of 400 double (which i clearly mentioned in my form). Please change this plan to 400 Double. It is your fault so i will not be bear any amount against this plan.,

Please find enclosed the receipt for your information.
