In the Name of Spouse column,

the entry will be DEBABRATA BASU in place of the existing entry DEBABRATA MITRA

In the Address column,

the entry will be SALT LAKE CITY in place of the existing entry SALT LAE

All other entries will remain unchanged.

Kindly advise the procedure I have to take for rectification.
